Shoreline Nutrition: Informed Consent

​

Working with a Registered Dietitian (RD) has both benefits and risks that are good to know about. Your RD will give you support and education about eating to care for a medical condition that has already been diagnosed by a doctor. Your RD cannot and will not diagnose or treat a medical condition. Your RD does not replace the care you get from a doctor. Some people may experience temporary changes in their gut and toilet habits. With any change in eating, your body may have to adapt. Changes in the amount of fiber you eat or what you drink may result in constipation, diarrhea, boating, or gas. 


If you have been diagnosed with Type 1 or Type 2 Diabetes, know about these risks:

           - Low blood sugar: Very low blood sugar is called hypoglycemia. When your blood sugar is too low it can make you pass out or even lead to death if not treated. Very low blood sugar can happen in people who take medicines for their diabetes such as insulin and some types of pills. Food, drink, and physical activity changes you make during your work with a RD may cause your blood sugar to go too low. Your diabetes medicines may need to be adjusted as you make changes in your food, drink, or physical activity.

           - High blood sugar: High blood sugar is called hyperglycemia. Very high blood sugar can make you pass out or even lead to death if not treated. When blood sugar is too high for too long it can lead to conditions called diabetic keto acidosis (DKA) or hyperosmolar hyperglycemic state (HHS). These need emergency treatment. DKA can also happen with blood sugars that aren’t high when taking certain diabetes pills, especially when making changes in what you eat or drink.

Please talk to your doctor and RD about any changes you make to your food and drink, medicines, and/or physical activity.

Health Conditions

 

There are some health conditions that make it unsafe and possibly life threatening to participate in virtual RD counseling. If you have any of these conditions or are at all unsure, please tell your RD.

 

         - Pregnant, nursing or pumping breast milk, or planning to get pregnant.

         - Type 1 or Type 2 Diabetes, and you inject insulin or use an insulin pump.

         - Any eating disorder such as Anorexia or Bulimia, now or in the past 5 years.

         - Heart Failure

         - Chronic Kidney Disease or Kidney Failure, with or without dialysis.

         - Liver Failure

         - Actively undergoing chemotherapy or radiation, or have in the last 6 months.

Not for Emergency Support

 

Shoreline Nutrition does not provide emergency services or crisis care. Shoreline Nutrition should not be used for any urgent care needs, medical emergency, or mental health emergency. 

Safety contacts: If there is a safety concern about you or others, Shoreline Nutrition may contact outside emergency services via 911, provide referrals to outside services, and/or contact your emergency contact person you have listed to help you connect to the proper medical care or support.

In case of emergency or crisis: If you need medical help right away please call 911 or go to the nearest hospital emergency room. If you are having a mental health crisis, call or text the free 988 Suicide & Crisis Lifeline at 988 or 800-273-8255.

 

Required by Law

 

Shoreline Nutrition takes the privacy of your protected health information seriously. Under HIPAA guidelines what you share with your RD is kept private, except when it must be shared under law, including:

         - With other health care providers to coordinate your care.

         - When there is reasonable suspicion of child, dependent, or elder abuse or neglect.

         - When you present as a danger to yourself or others.

         - Other circumstances as required by state or federal law.

 

Terms and Privacy

 

Shoreline Nutrition will keep your personal health information private. Shoreline Nutrition may share personal health information with your other healthcare providers as needed to coordinate care, collect payment, or perform standard operations such as technical platform management.

 

Consent

 

By scheduling an appointment with a Shoreline Nutrition RD you are consenting to the above information. If you have any questions or concerns please notify your RD at the start of your appointment.
